When you send a signature request, each signer will receive an email:
If you have uploaded a company logo, it will appear at the top of the email, as well as the top of the signing page (below).
After clicking on the link they come to a mobile optimised signing page like this:
The 'Confirm Identity' area only shows if you have opted for second factor proof of Signer Identity.
The Signer's name is always requested and any other fields (eg 'Address' in this example) are those you left placeholders for.
To preview the document, Signers click 'Show/Hide Document':
As the signer fills in information, it appears on the document preview.
And then they sign...
